This official name for this item is "Diamond Jammer Snaps Plaque".
Besides the fact that this is no ordinary plaque, we don't have much information on this strange item. We don't know if it is unreleased, glitched, or even possibly a hacked item. AJHQ may make a statement about this item sometime in the near future, but for now, all we know is that this plaque, as of today, at this time, is very, very rare.
The video above shows Invulnerable trading dragimiam2 for this item. It is unknown how or when Dragi got the plaque.
There a few main theories on how you can obtain this plaque, though:
1) win a DE contest more than once
